I tried to submit a support ticket on mainwp.com/my-account. I filled out all the fields and hit submit. It came back saying there was a problem and check out required fields. There were error messages about 2 additional fields now showing that were not on the original form (enter URL for dashboard and child site). I’d be happy to enter this info, but there is no input box to enter it, only the error message. Which makes the whole form unsubmitable.

It sounds like you may have accidentally pasted the System Reports directly into these two fields.
When you do that, you will get an error message about a problem with the submission.
The System Report should be uploaded to a a text-sharing platform like Pastebin, GitHub, Gist, and only the URL should be entered in those two fields.
